Withdrawal/Resignation Checklist - Form Completion Steps

The form must submitted by 11:59 P.M. on the deadline date to withdraw or resign from the University with a grade of W. (See Academic Calendar). 

Steps to complete the form:
1.Verify your name, ULID, level, college, major, and classification.
2. Enter a contact phone number.
  • Note: This is not a formal update to your phone number.  
3Select "Drop a Class" or "Drop all Classes (Resign)."
4. Enter Reason for Drop by choosing one of the options in the drop drown list.
  • Note: If you choose military, you must upload a copy of your military orders. If you choose other, you may utilize the free text box to input any additional information. 
5. If dropping a class(es), select the class(es).
  • Note: If dropping all classes (resign), skip to Step 6. 

6. Click to acknowledge and enter your name.
7. Click the Submit Form button at the bottom of the page.
8. Final confirmation needed to fully submit the form.
